You are entitled by law to have access to the medical information we hold. Please apply in writing to the practice manager. A fee may be charged together with the cost of any photocopies if required. For more information ask for a leaflet at reception.

From time to time, information about your health may be shared with other health care professionals involved in your treatment. More information on the data we hold and what it is used for can be found in the NHS leaflet entitled 'Your Medical Record' which is available from reception.

The practice holds computerised health records and is registered under the Data Protection Act.
